TRANSITIONAL CARE UNIT

Licensed by California Dept. of Health Care Services to offer physician individualized skilled nursing care in a warm nurturing environment for medically fragile children.

Through the Transitional Care Unit, your child over 18 years old can receive skilled nursing care while attending our adult day program. Clients in our TCU attend on average 40 hours per week and participate in our overnight respites.

Facility operating hours vary based on parent need but generally are from 8:00 a.m. to 6:00 p.m. Respites also vary based on parent's need but generally are a few weekends a month.
